The Town of Lincoln Parks and Recreation Department is soliciting bids for the drilling of a Bedrock Well at the Codman Athletic Field site located on Ballfield Road, Lincoln, MA.
NOTE* - RFP Price Sheet modified on Tuesday, Sept 17 at 2:30PM to reflect a single-well project. Some language in the price sheet erroneously referred to two wells in the original documents. Unit pricing will be used to compare bids, so this change has no substantive impact on bid results.

1. Entity awarded to perform CONTRACT WORK described in SPECIFICATIONS having valid registration with Massachusetts Department of Environmental Protection (DEP), Division of Water Resources to drill wells.
2. Entity having minimum five (5) years relevant experience of size and type of CONTRACT WORK required by SPECIFICATIONS, and acceptable to OWNER’S REPRESENTATIVE.
3. CONTRACTOR shall employ persons supervising and constructing proposed CONTRACT WORK required by SPECIFICATIONS to be experienced in bedrock well construction for three (3) years minimum.
4. CONTRACTOR shall furnish to ENGINEER references for previous relevant work on Bid Sheet at end of SPECIFICATIONS including project names and addresses, contact persons and information of five (5) similar bedrock well projects over past three (3) years.

| 5. Because the works sites are located on the Lincoln Public School Campus, CONTRACTOR shall only employ persons supervising and constructing proposed CONTRACT WORK required by SPECIFICATIONS who are able to satisfactorily pass a CRIMINAL OFFENDER RECORD INQUIRY (CORI) conducted by the Town of Lincoln (Sample Attached). All CORI applications and results must be reviewed by the Town prior to commencement of work.
